4-Year-Old Ismaeel Knights presented to the Georgetown Public Hospital (GPHC) Pediatric Unit with symptoms and signs of a urinary tract infection for which he was admitted. However, an ultrasound was performed and revealed bilateral hydronephrosis (the swelling of both kidneys due to a build-up of urine).

Further evaluation by his medical team at GPHC, with dilatation and radiology imaging of the prostatic urethra, suggests he has Posterior Urethral Valves (PUV). PUV are obstructive membranes that develop in the urethra (tube that drains urine from the bladder).  The valve can obstruct or block the outflow of urine through the urethra. When this occurs, the bladder, ureters and kidneys become progressively dilated, which can lead to damage.

Based on these findings, Ismaeel urgently needs posterior urethral valves ablation, which is not currently available in Guyana. As a result, he was referred to Dr. Barbara Rampersaud, a Urologist in Trinidad, who specializes in this procedure. Dr. Rampersaud has reviewed Ismaeel’s case and has agreed to treat him in Trinidad.

Update: June 14, 2023

Ismaeel arrived safely in Trinidad and will meet with his medical team to discuss treatment plan and have the necessary preliminary testing, followed by surgery in the coming days.


Update: June 21, 2023

Ismaeel underwent successful posterior urethral valves ablation surgery on Thursday, June 15. He remained under close observation by his medical team in Trinidad and was cleared for discharge a few days later.

He returned home to Guyana today, into the loving arms of his father. He no longer needs a catheter and is fairly pain free which he is very happy about. This energetic little guy will now have follow-up care and monitoring by his local medical team in Guyana to ensure he continues to progress well.
